Illinois has a program to help individuals who have absolutely no means of funding an end-of-life service. “Public assistance” services are funded through the Illinois Department of Human Services. The deceased must have an active medical case with the state to qualify. We will contact the Department of Human Services to see if the deceased qualifies and seek “conditional” approval. Our funeral director will make funeral arrangements and discuss the options that fit in the guidelines of our provided program. We will ask you to sign a contract, which is a promise to pay, in the event that the deceased does not qualify for a public assistance funeral, or they are denied for any reason. If the payment request from the Department of Human Services is rejected, you will be responsible for the balance due.

•Basic services of funeral director and staff- including removal from place of death (within 25 miles of Du Quoin), arrangements conference, and filing necessary legal paperwork
•Obituary posted on Maxton-Rosado Funeral Home website (maxtonrosado.com) and Facebook page for sharing
•Coroner’s Permit to Cremate
•One Certified Death Certificate
•Basic Plastic Temporary Cremation Urn
•A two hour visitation held prior to the service on the same day
•Memorial services held at the funeral home
•Basic Register Book Package (includes guest book, 100 folders, 25 thank you cards, and video tribute)
•Committal Service at Cemetery (includes transportation of cremains to cemetery and licensed director)
•Temporary Grave Marker will be provided for cemetery. A monument is not included.
There are things not covered by the Department of Human Services that families may choose to add. These items, if selected, must be paid in advance to funeral services.
